The following download is a legally acceptable form to designate a guardian for a minor in Texas. It is heavily annotated to help explain to you what each item means.
The designation of guardian can be freestanding, but it should be done in conjunction with a will, which allows you, the parent, to also create a trust for the money and property that you have to leave for your children. Leaving property to your children in trust allows you to specify how the money is to be used, whether it is to be preserved for the future, and when the children can have direct access to the money (they can legally recieve it when they turn 18, but many parents leave the Trust in place well beyond that age).
